- CagriSema (cagrilintide plus semaglutide) is an investigational Novo Nordisk product, not FDA-approved as of 2 October 2026. Registered phase 3 trials: REDEFINE 1 (NCT05567796), REDEFINE 2 (NCT05394519), REDEFINE 3 (NCT05669755, cardiovascular outcomes, no results found).
- REDEFINE 1 and 2 are the pivotal obesity papers (NEJM 2025). The REIMAGINE 1-3 type 2 diabetes phase 3 trials and REDEFINE 5 (Japan and Taiwan) were published in 2026.
- Half-life values are from the phase 1b co-administration trial (cagrilintide 159-195 h; semaglutide 2.4 mg 145-165 h). A fixed-dose CagriSema pharmacokinetic study was not found.
- No evidence was extracted for liver health, cholesterol or osteoporosis.

Weight results in obesity trials
REDEFINE 1 found a mean weight change of -20.4% with CagriSema against -3.0% with placebo at week 68, in adults without diabetes.[7]
In REDEFINE 5 (Japan and Taiwan) the combination reached -18.4% against -11.9% with semaglutide over 68 weeks.[1]
In REDEFINE 1 the combination group was also more likely than placebo to reach weight-loss targets of 5% or more, 20% or more, 25% or more and 30% or more.[7]
In a post hoc analysis, 30.3% of participants on CagriSema reached both BMI and waist-to-height targets, against 3.3% on placebo.[6]
Results in type 2 diabetes
In people with type 2 diabetes in REIMAGINE 1, weight fell 13.8% at 2.4 mg each against 1.4% for placebo at 40 weeks.[2]
REIMAGINE 2 reported a baseline HbA1c of 8.2%, and the combination beat semaglutide 2.4 mg by 0.16 percentage points.[3]
Earlier-phase timepoints
In the phase 2 trial the HbA1c change at week 32 was -2.2 percentage points for CagriSema against -0.9 for cagrilintide.[9]
Common questions
How long does CagriSema take to work?
The studies we summarise measured outcomes at set points, for example Coprimary endpoints: relative change in body weight and 5% or more weight reduction at 68 weeks; Primary endpoints: percent change in body weight and 5% or more weight reduction at 68 weeks. Timelines outside studies vary.
Do you show CagriSema before and after photos?
No. Photos can't be verified, rarely show dose, diet or other drugs, and only the most dramatic ones get posted. We summarize measured outcomes instead.
