Claimed benefits and what studies show
Each row is an area where people claim a benefit, with the strongest study we summarise behind it. Animal and cell results are labelled and are not human results. User reports are not counted as evidence here.
| Claim | Best evidence | Studies (human / animal) | What was measured | Page |
|---|---|---|---|---|
| Weight loss | Human RCT evidence, 4 of 4 | 2 / 1 | Body weight at week 26 vs placebo; Cagrilintide 4.5 mg vs liraglutide 3.0 mg; Body weight at week 20 (with semaglutide 2.4 mg) | Evidence for weight loss |
| Heart health | Human RCT evidence, 4 of 4 | 1 / 0 | QTc interval; Body weight with cagrilintide monotherapy | Evidence for heart health |
Strongest study per claim: Weight loss, Lancet 2021, PMID 34798060; Heart health, Diabetes, obesity & metabolism 2024, PMID 39279639.
What the studies measured
| Outcome | Result | Study type | Source |
|---|---|---|---|
| Pharmacokinetics in renal and hepatic impairment | Cagrilintide total exposure and peak concentration were similar across renal and hepatic function groups; the authors conclude dose adjustment is not warranted. | Human controlled trial | Clinical pharmacokinetics 2026, PMID 42228334 |
| QTc interval | No clinically relevant QTcF prolongation after the last 4.5 mg dose; upper 90% confidence limits of the placebo-adjusted change were below 10 ms. | Human RCTn = 105 | Diabetes, obesity & metabolism 2024, PMID 39279639 |
| Body weight at week 26 vs placebo | Weight reductions were 6·0%-10·8% across cagrilintide doses 0·3-4·5 mg versus 3·0% with placebo (trial product estimand). | Human RCT | Lancet 2021, PMID 34798060 |
| Cagrilintide 4.5 mg vs liraglutide 3.0 mg | Cagrilintide 4·5 mg gave a 10·8% weight reduction versus 9·0% with liraglutide 3·0 mg. | Human RCT | Lancet 2021, PMID 34798060 |
| Body weight at week 20 (with semaglutide 2.4 mg) | Combined with semaglutide, body weight fell by 15·7% (cagrilintide 1·2 mg) and 17·1% (2·4 mg) versus 9·8% with pooled placebo at week 20. | Human RCTn = 96 | Lancet 2021, PMID 33894838 |
| Body weight with cagrilintide monotherapy | Cagrilintide monotherapy reduced body weight by a mean difference of -6.08% (-5.89 kg) and also reduced blood pressure, without meaningful HbA1c improvement. | Meta-analysisn = 5425 | Annals of medicine and surgery 2026, PMID 42583410 |
| Body weight in diet-induced obese mice | Wild-type cagrilintide-treated mice lost -3.4 g of body weight. | Animal · mouse (not a human result) | EBioMedicine 2025, PMID 40609154 |
| Receptor dependence | Cagrilintide's weight-lowering effect depended on the amylin receptors AMY1R and AMY3R in mice. | Animal · mouse (not a human result) | EBioMedicine 2025, PMID 40609154 |
| Receptor pharmacology | AM833 (cagrilintide) showed a distinct pharmacological profile across receptor binding, activation and regulation measures compared with other agonists. | In vitro (not a human result) | The Journal of pharmacology and experimental therapeutics 2021, PMID 33727283 |
The weight-loss claim and the trial behind it
Claim: cagrilintide lowers body weight. In a phase 2 trial in adults without diabetes, every weekly dose from 0.3 to 4.5 mg beat placebo on percentage weight change, by the trial's own adherent-participant analysis.[3]
Claim: it works better than an approved daily injection. The same trial found the top 4.5 mg weekly dose ahead of liraglutide 3.0 mg, with a modest gap.[3]
Pooled data point the same way: a meta-analysis of randomised trials reports average weight and blood-pressure falls for cagrilintide on its own, with no meaningful HbA1c change.[5]
Blood sugar, blood pressure and heart claims
Claim: it helps blood sugar. In a 32-week phase 2 trial in type 2 diabetes, the cagrilintide-alone arm fell by 0.9 percentage points in HbA1c, less than the combination arm.[13]
Claim: it is safe for the heart. A thorough QT study in healthy volunteers found no clinically relevant effect on the QTcF interval after the last 4.5 mg dose.[2]
The meta-analysis authors describe the overall picture as weight loss plus cardiometabolic improvement, and ask for longer trials.[5]
Claims that only have animal data
Claim: it cuts appetite through brain amylin receptors. This rests on a mouse study: high-fat-fed mice given cagrilintide ate less in the first days than vehicle-treated mice, and the effect depended on receptors the knockout mice lacked.[6]
Receptor-assay work (cell-based, not in people) classes the molecule as a non-selective agonist of amylin and calcitonin receptors, which is the proposed route for its effect.[7]
What the literature does not cover
As of the PubMed search dated 2026-10-02, only 13 records matched cagrilintide, semaglutide and the randomised-controlled-trial publication type, so claims beyond weight and glycaemic outcomes have little randomised evidence behind them.
Calculated from search query cagrilintide[tiab] AND semaglutide[tiab] AND "Randomized Controlled Trial"[pt], date 2026-10-02, count 13; source web/src/data/evidence/cagrilintide.json searches. Formula: count = records returned by the dated PubMed query

Common questions
Is Cagrilintide proven to work?
The strongest study we summarise for Cagrilintide is human rct evidence, among 8 studies. A trial result is not an approval; see the legal status.
What does cagrilintide do?
In the published human trials cagrilintide is tested as a once-weekly injection for weight management, and the best-supported finding is weight reduction against placebo.[3]
What is cagrilintide peptide used for?
It is an investigational amylin analogue; the cited trials study it for weight management and, combined with semaglutide, for type 2 diabetes.[1]
