Evidence for liver
Each row is an outcome related to liver from a published study, strongest design first. Animal and cell results are labelled and are not human results. User reports are not counted as evidence here.
| Outcome | Result | Study type | Source |
|---|---|---|---|
| Liver fat at 24 weeks (primary) | Liver fat changed by -82.4% with 12 mg and -42.9% with 1 mg, versus +0.3% with placebo at 24 weeks. | Human RCTn = 98 | Nature medicine 2024, PMID 38858523 |
| Normal liver fat (<5%) | Normal liver fat was reached by 86% on 12 mg and 27% on 1 mg versus 0% on placebo. | Human RCTn = 98 | Nature medicine 2024, PMID 38858523 |
| Link to weight, abdominal fat and insulin sensitivity | Liver fat reductions were significantly related to changes in body weight, abdominal fat and metabolic measures (an association, not a separate abdominal-fat result). | Human RCTn = 98 | Nature medicine 2024, PMID 38858523 |
| Steatohepatitis markers | In female mice, retatrutide over the final 2 weeks of a 31-day model reduced body weight, ALT, liver triglycerides and cholesterol, and liver inflammatory markers versus vehicle. | Animal · mouse (not a human result) | American journal of physiology. Gastrointestinal and liver physiology 2025, PMID 41056349 |

How strong is it?
A human randomized trial outweighs any number of animal studies. Animal and cell results show a mechanism worth testing, not that it works in people.
The liver, fatty liver and the retatrutide substudy
MedlinePlus describes the liver as the largest organ inside the body, helping to digest food, store energy and remove poisons.[6]
The retatrutide liver substudy looked at participants of the phase 2 obesity trial who had fatty liver disease and at least 10% liver fat.[4]
How much liver fat changed
At 24 weeks mean liver fat changed by -81.4% on 8 mg and -82.4% on 12 mg, against +0.3% on placebo.[4]
Normal liver fat (under 5%) was reached by 79% on 8 mg and 86% on 12 mg, and by 0% on placebo.[4]
The authors link liver-fat reductions to changes in body weight, abdominal fat and metabolic measures of insulin sensitivity and lipid metabolism.[4]
In female mice on a steatohepatitis diet, 2 weeks of retatrutide reduced body weight, ALT, liver triglycerides, cholesterol and inflammatory markers; this is animal data.[5]
How much liver research exists
A PubMed search of retatrutide with liver terms (2026-10-02) found 18 human-only papers and 4 under the animal filter; 1 carried a randomized-trial tag.
Calculated from pubmed query (retatrutide[tiab] OR LY3437943[tiab]) AND (liver[tiab] OR hepat*[tiab] OR steato*[tiab] OR NAFLD[tiab] OR NASH[tiab] OR MASLD[tiab] OR MASH[tiab] OR cirrho*[tiab]); date 2026-10-02; human only count 18; animal filter count 4; randomized filter count 1. All peptides for liver, comparedCommon questions
Does Retatrutide help with liver?
The strongest study we summarise for liver is human rct evidence, across 2 studies with an outcome for liver. A trial result is not an approval; see the legal status.
Is Retatrutide approved for liver?
No. Retatrutide is not FDA approved for any use, including liver.
What is the best peptide for liver?
There is no single answer. Our liver overview compares each candidate by evidence grade and legal status rather than by popularity.
Does retatrutide reduce fatty liver?
In the phase 2 liver substudy, mean liver fat fell from baseline in every retatrutide group, and the 12 mg group reached -82.4% at 24 weeks.[4]
Can retatrutide damage the liver?
The liver substudy's stated objective was to measure liver fat; its abstract does not report liver injury as an outcome.[4]
