The weight-loss claim
Claim: it produces large weight loss. In SURMOUNT-1 the mean weight change at week 72 was -15.0% (5 mg), -19.5% (10 mg) and -20.9% (15 mg), against -3.1% for placebo.[29]
Claim: it beats semaglutide. In SURMOUNT-5, weight changed by -20.2% with tirzepatide against -13.7% with semaglutide, in an open-label trial.[3]
Claim: the weight stays off. SURMOUNT-4 found weight regain after withdrawal while continued treatment maintained and augmented the initial reduction.[8]
The blood sugar claim
Claim: it lowers HbA1c. In SURPASS-1 mean HbA1c decreased by 1.87%, 1.89% and 2.07% at 5, 10 and 15 mg, against +0.04% with placebo.[30]
In SURMOUNT-2, people with obesity and type 2 diabetes lost -12.8% (10 mg) and -14.7% (15 mg) of body weight against -3.2% with placebo.[31]
Sleep apnea, liver and heart claims
Claim: it helps sleep apnea. In the first SURMOUNT-OSA trial the apnea-hypopnea index fell by 25.3 events per hour with tirzepatide against 5.3 with placebo.[9]
The OSA trials' authors also report reductions in hsCRP concentration and systolic blood pressure compared with placebo.[9]
Claim: it helps fatty liver. A post hoc analysis found significant reductions in NASH biomarkers at higher doses in type 2 diabetes.[32]
Claim: it protects the heart. A pooled analysis of the SURPASS programme found a hazard ratio of 0.80 for four-component MACE, with a confidence interval of 0.57-1.11 that does not exclude no difference.[33]
Hormones and muscle claims
Claim: it raises testosterone in men with obesity. A controlled pilot in men with obesity and metabolic hypogonadism reported higher testosterone after 2 months of treatment than the comparison groups.[7]
Claim: it preserves muscle. In a SURMOUNT-1 substudy about 75% of the weight lost was fat mass and 25% lean mass, the same split as placebo.[6]

Common questions
Is Tirzepatide proven to work?
The strongest study we summarise for Tirzepatide is human rct evidence, among 30 studies. A trial result is not an approval; see the legal status.
Is tirzepatide proven to work for weight loss?
In the cited randomised trials it beat placebo and semaglutide on weight change; the weight results above are from SURMOUNT-1 and SURMOUNT-5.[3]
