How long it acts
Half-life values from FDA labels and published studies, by route. Animal values are labelled and do not describe people.

The receptors tirzepatide acts on
Both the Mounjaro and Zepbound labels describe tirzepatide as a GIP receptor and GLP-1 receptor agonist that selectively binds to and activates both receptors.[1]
The Zepbound label adds that GLP-1 is a physiological regulator of appetite and caloric intake, and that nonclinical studies suggest GIP may further contribute.[2]
In animals, the Zepbound label says tirzepatide distributes to and activates neurons in brain regions involved in regulating appetite and food intake.[2]
What it does in the body, per the labels
The Mounjaro label says tirzepatide enhances first- and second-phase insulin secretion and reduces glucagon levels, both in a glucose-dependent manner.[1]
It reduced fasting glucagon concentration by 28% and glucagon AUC after a mixed meal by 43% at 15 mg after 28 weeks, against no change for placebo.[1]
The label states that tirzepatide delays gastric emptying, largest after the first dose, with the effect diminishing over time.[1]
The Zepbound label says tirzepatide lowers body weight with greater fat mass loss than lean mass loss, and that the effects are likely mediated by affecting appetite.[2]
Why it is a weekly injection
The labels explain that a C20 fatty diacid enables albumin binding and prolongs the half-life.[1]
Steady-state plasma concentrations were reached after 4 weeks of once-weekly dosing, and exposure rose in proportion to dose.[2]
Mechanism checks in human trials
In SURPASS-2, tirzepatide at all doses was noninferior and superior to semaglutide on HbA1c, a comparison against a selective GLP-1 receptor agonist.[4]
An MRI substudy found liver fat fell by 8.09% with pooled tirzepatide 10 and 15 mg against 3.38% with insulin degludec.[5]

Common questions
How long does Tirzepatide act in the body?
The half-life values we found are approximately 5 days (subcutaneous); approximately 5-6 days (subcutaneous). A half-life is how long the amount in the body takes to fall by half; it is not the same as how long an effect lasts.
