The hormone axis it acts through

The authors of the 12-man phase IIB study say that concomitant changes in LH and FSH suggest normalisation of endogenous testosterone production through the hypothalamic-pituitary-testicular axis.[6]

Enclomiphene and transdermal testosterone both raised total testosterone early in treatment but had opposite effects on FSH and LH.[7]

The phase III authors report that enclomiphene raised FSH and LH while the testosterone gel lowered them.[8]

Its place among clomiphene isomers

Clomiphene is a mixture of two isomers, zuclomiphene and enclomiphene, and enclomiphene is the isomer evaluated here.[6]

In male mice (animal data), the chronic-dosing paper found harmful effects on reproductive tissues linked to zuclomiphene only, and argued for developing the single isomer.[9]

Timing

After stopping treatment, the effects on LH and TT persisted for at least a week.[7]

The same study found no temporal association between peak drug levels and the peaks of LH or TT.[7]

Total testosterone fell back to pretreatment values one month after treatment ended in the 12-man study.[6]
