Claimed benefits and what studies show
Each row is an area where people claim a benefit, with the strongest study we summarise behind it. Animal and cell results are labelled and are not human results. User reports are not counted as evidence here.
| Claim | Best evidence | Studies (human / animal) | What was measured | Page |
|---|---|---|---|---|
| Testosterone | Human RCT evidence, 4 of 4 | 5 / 1 | Total testosterone; Sperm concentration vs gel; Testosterone, LH, sperm | Evidence for testosterone |
Strongest study per claim: Testosterone, BJU international 2016, PMID 26496621.
What the studies measured
| Outcome | Result | Study type | Source |
|---|---|---|---|
| Total testosterone | Total testosterone rose from baseline at 16 weeks in all groups, including enclomiphene. | Human RCT | BJU international 2016, PMID 26496621 |
| Sperm concentration vs gel | Sperm concentration stayed in the normal range with enclomiphene but fell markedly with testosterone gel. | Human RCT | BJU international 2016, PMID 26496621 |
| Testosterone, LH, sperm | Enclomiphene raised morning testosterone, estradiol and LH similarly to topical gel; sperm counts conserved. | Human RCT | Fertility and sterility 2014, PMID 25044085 |
| Total testosterone at 6 months | At 6 months total testosterone was 545 (gel) and 525 (enclomiphene) pg/dL. | Human RCTn = 12 | The journal of sexual medicine 2013, PMID 23530575 |
| Sperm counts | Sperm counts rose in seven out of seven men at 3 months and six out of six men at 6 months on enclomiphene. | Human RCTn = 12 | The journal of sexual medicine 2013, PMID 23530575 |
| Total testosterone at day 42 | At 25 mg daily, day-42 testosterone averaged 604 ng/dL versus 500 ng for transdermal testosterone. | Human RCTn = 48 | BJU international 2013, PMID 23875626 |
| Onset | Testosterone increased within two weeks on enclomiphene. | Human RCTn = 48 | BJU international 2013, PMID 23875626 |
| Median testosterone change | Median testosterone increase was 166 with enclomiphene versus 98 ng/dL with clomiphene (P=0.20, not significant). | Human observationaln = 66 | Translational andrology and urology 2024, PMID 39434750 |
| Total testosterone vs placebo | SERMs (clomiphene or enclomiphene) raised total testosterone by a mean 273.76 ng/dL versus placebo, with high heterogeneity (I2 = 89%). | Meta-analysis | Archives of endocrinology and metabolism 2025, PMID 41066380 |
| Versus T gel | No significant difference in total testosterone between SERMs and T gel. | Meta-analysis | Archives of endocrinology and metabolism 2025, PMID 41066380 |
| Testosterone with enclomiphene | Pooled testosterone rose by a mean 7.50 nmol/L with enclomiphene (and 11.56 nmol/L with clomiphene). | Meta-analysisn = 292 | Andrology 2023, PMID 36604313 |
| Testicular effects of isomers | Isolated enclomiphene raised testosterone production with no change in testicular histology in male mice. | Animal · mouse (not a human result) | BJU international 2016, PMID 26220499 |
The testosterone claim
Claim: it raises testosterone. In two parallel phase III trials in overweight men with secondary hypogonadism, total testosterone rose over 16 weeks in all groups, including both enclomiphene doses.[1]
A phase II dosing study shows the effect size: mean total testosterone reached 604 ng/dL on the highest enclomiphene dose and 500 ng/dL on transdermal testosterone, not different from each other.[4]
A meta-analysis of randomised trials pooled clomiphene and enclomiphene as one SERM group and reports a mean difference in total testosterone of 273.76 ng/dL against placebo; it does not isolate enclomiphene.[6]
The fertility and sperm claim
Claim: it keeps sperm counts up while raising testosterone. In the phase III trials, enclomiphene kept sperm concentration in the normal range while the testosterone gel group showed a marked drop in spermatogenesis.[1]
In a small phase IIB study of men previously on topical testosterone, enclomiphene raised sperm counts in seven out of seven men at 3 months.[3]
Obesity-related low testosterone and side-effect claims
Claim: it works in men with obesity. A pooled analysis found a testosterone increase with enclomiphene of 7.50 nmol/L.[7]
Claim: fewer side effects than clomiphene. A retrospective review of 66 patients found significantly fewer adverse effects after switching to enclomiphene; it was not a randomised trial.[5]
Animal-only claim
Mouse data (animal only) found that the enclomiphene isomer had positive effects on testosterone production and no effects on testicular histology, while zuclomiphene did not.[8]
What no regulator has approved
The Drugs@FDA lookup of 2026-10-02 returned no match for enclomiphene, so none of these benefits is a labelled indication.[14]

Common questions
Is Enclomiphene proven to work?
The strongest study we summarise for Enclomiphene is human rct evidence, among 9 studies. A trial result is not an approval; see the legal status.
What is enclomiphene used for?
The studies cited test it in men with secondary hypogonadism, to raise testosterone without the suppression of sperm production seen with testosterone gel.[2]
